Sports Physicals for 9th through 12th grade are now valid for 2 years. Elementary students still need a physical every year to participate in sports.

If your 9th to 12th grade student received a physical last school year 2025-2026, you only have to complete this MHSA Interim Medical Form this year.

It is also located under Parents > Forms > Students Forms right next to the 2026-2027 Sports Physical forms.
